MedPAC Proposes Alternative to MIPS

The Medicare Payment Advisory Commission (MedPAC) proposed an alternative to the Merit-based Incentive Payment System (MIPS) at their October 5, 2017 public meeting. This proposal expanded on their proposal to redesign MIPS found in their June 2017 Report to Congress.


MedPAC proposes to eliminate MIPS and its related reporting requirements. MIPS would be replaced by a voluntary value program. Under the proposed model all clinicians would have a portion of fee schedule payments withheld. Clinicians then could:

  • elect to be measured with a large entity of clinicians and be eligible for a value payment;
  • elect to join an Advanced Alternative Payment Model and receive withhold back; or
  • make no election and lose the withhold.


MedPAC believes that MIPS is not sustainable and that since CMS will begin making payment adjustments in 2019, action is needed immediately.
